Hospice Nurse Job Summary

The Hospice Nurse will be responsible for assessing and evaluating the patient’s physical, psychological, and mental needs and providing nursing services aimed at improving the quality of life for both the patient and their family. The Hospice Nurse will work collaboratively with other members of the healthcare team to ensure that the patient’s wishes and needs are respected.

Responsibilities

  • Assess and evaluate the patient’s physical, psychological, and mental needs
  • Educate the patient and their family on end-of-life care
  • Develop and implement a plan of care that addresses the patient’s needs
  • Monitor the patient’s progress and adjust the plan of care as needed
  • Maintain patient records and provide written reports on the patient’s condition

Required Skills and Qualifications

  • A servant's heart for patients and families with a passion for providing the highest quality patient care.
  • Education: Graduate of an accredited school of nursing with an Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN).
  • License: Active Registered Nurse (RN) license issued by the State Board of Nursing. Multistate licensure preferred.
  • Certification: Certified Hospice and Palliative Nurse is a plus
  • CPR Certification:Current CPR certification is required
  • Experience: 1+ years of nursing experience in a clinical care setting (hospice or end-of-life experience preferred)

Social Worker (Hospice) Job Summary:

The Hospice Social Worker provides compassionate and holistic care to patients and families facing end-of-life, offering emotional support, counseling, and resource coordination to enhance quality of life during a challenging time.

Responsibilities:

  • Assessments:

Conduct comprehensive psychosocial assessments of patients and families to identify their emotional, social, and spiritual needs.

  • Care Planning:

Develop and implement individualized care plans that address patient and family needs, collaborating with the interdisciplinary team.

  • Counseling & Support:

Provide counseling and support services to patients and families, including grief counseling and bereavement support.

Required Skills and Qualifications:

  • Education: Master's Degree in Social Work (MSW) from an accredited university.
  • Licensure: Licensed Masters or Clinical Social Worker (LMSW or LCSW) in current state, multi state licensure preferred.
  • CPR Certification: Current CPR certification is required
  • Experience: Experience working in hospice or palliative care settings preferred.

Chaplain (Hospice) Job Summary

A Hospice Chaplain provides spiritual and emotional support to patients, families, and healthcare teams during the end-of-life journey. They assess spiritual needs, conduct religious services, offer counseling, and collaborate with other healthcare professionals to ensure comprehensive care.

Key Responsibilities:
