Overall Purpose

Are you a compassionate mission-driven person looking to make a meaningful impact on the lives of complex medical patients? As a local outreach associate at P3 Health Partners, you’ll bring hope and healing to patients with complex medical needs. In this full-time role, you’ll play a vital role as a team member, transforming lives and empowering primary care physicians to deliver exceptional care to those who need it most.

As a local outreach associate, you will contact target patients, explain the P3 Health Partner Program to patients, or their contacts, and schedule these patients for visits in a variety of settings. You may assist in rescheduling patients. Communication with the P3 teams will ensure smooth transitions throughout the patient’s health care journey.

This position will be in a P3 Health Partners facility. This is not a remote position.

Essential Functions

  • Perform telephonic outreach to patients following a standardized operational process.
  • Work collaboratively with P3 Health Partner teams and their affiliates.
  • Update and maintain accurate patient contact information.
  • Ensure patients with which you interact have a solid understanding of the P3 Health Partner offerings as well as a continued relationship with their primary care physicians.
  • Accurately schedule patients for visits in a variety of locations.
  • Adhere to the local outreach role-based handbook making recommendations for improvement when identified.
  • Serve as a positive and collaborative team member.
  • Uphold high standards of excellence and professionalism.
  • Respond positively to make any necessary adjustments to case reviews.
  • Confidently use technology to positively impact outcomes.
  • Ensure patient privacy and adherence to all HIPAA regulations and standards for handling PHI.
  • Participate in quality improvement initiatives and operational efficiency.
  • Maintain compliance with required trainings, meeting attendance, timely responses to emails, and other requirements.
  • Role model the P3 Health Partners core values.

Qualifications

  • High school completion
  • Experience in a service industry role preferred
  • Flexible, self-driven, collaborative and innovative personality
