Overall Purpose

As the National Outreach Director at P3 Health Partners, you’ll bring hope and healing to patients with complex medical needs. In this full-time role, you’ll be a vital leader, transforming lives and empowering primary care physicians to deliver exceptional care to those who need it most.
As the Director, National Outreach, you will develop strategy, playbooks, role-based handbooks and education guiding patients to their care teams or other resources to achieve their health care goals. You will design objective KPI dashboards. Local outreach teams will report to you directly while other roles will be supported by your actions.
Travel may be required to achieve your goals. You collaborate closely with the executive and regional leadership teams.
Essential Functions
- Lead outreach team members as a cohesive unit.
- Work collaboratively with teams under the P3 Health Partners organizational structure.
- Effectively manage outreach KPIs.
- Develop outreach standardized playbook.
- Develop role-based handbooks for team members.
- Serve as a positive and collaborative leader and team member.
- Uphold high standards of excellence.
- Develop standardized outreach strategies with adaptations for local market nuances.
- Perform random and targeted team member outreach reviews ensuring integrity of patient care.
- People management team members.
- Educate P3 team members during onboarding and as necessary.
- Confidently use technology to positively impact outcomes.
- Ensure patient privacy and adherence to all HIPAA regulations and standards for handling PHI.
- Participate in quality improvement initiatives and operational efficiency.
- Maintain compliance with required trainings, meeting attendance, timely responses to emails, and other requirements.
- Role model the P3 Health Partners core values.
Qualifications
- Minimum of 5 years leadership experience preferred
- Minimum of 5 years outreach strategy experience preferred
- Experience in geriatric outreach preferred
- Flexible, self-driven, collaborative and innovative personality
Travel
This position is virtual with travel to meetings and markets as requested or necessary to meet goals.
